Showcase of Inquiry Forms for Design Services
Published in GalleriesOne of the main goals of portfolio websites is to attract inquiries from potential clients. Most portfolio sites use a contact form as the primary method for collecting those inquiries. While forms are a rather standard part of a website design, some freelance designers and agencies have done a great job of creating forms and contact pages that are visually appealing.
In this post we’ll showcase portfolio sites with contact/inquiry forms that may serve as inspiration for your own design work.

Thanks for featuring our work!
Indeed, the main target of a website is to generate inquiries from potential clients. For that queries a form is required. An attractive as well as easy form will make the customer glued to the site. So the very important to give preference to the inquiry form from designing point of view. This post contains a gem of collection of query forms. Thanks for sharing the collections.
This is a nice idea for a post. I really love the Solid Giant form, it’s good to give the client an idea of what budgets you work with right from the start.
Leonid,
You’re welcome. I love your site’s design, it looks great.
It’s funny how most people don’t realize what a great tool a lead gathering form can be for them. One of the things that I’ve learned over the years is to differentiate between the type of forms used on the site. Forms with a number of entry fields are usually in place when the site has already established a customer’s course and expects the customer to spend time filling out the information in receipt of something valuable to the customer (like a loan app, or the receipt of an informational packet). To gather leads you want to keep it short and sweet. One to three fields maximum and never with invasive requests.
it’s so inspiring…
In my line of work, we live and die by the leads generated from our request information forms. What has surprised me in our testing is that the most attractive form does not necessarily warrant the highest number of conversions. What would put the cherry on top of this list of excellent form designs is a % of conversions – how many people who went to this page completed the form? Or how many people started the form and actually submitted it?
Bookmarked to revisit the next time we start testing our forms again. Good stuff.
It’s funny how the forms can vary in length so much. My personal favourite is the Solid Giant, it’s creative and memorable for all of the right reasons.
